Lead Functional Medicine Physician
Dr. Jaclyn Tolentino
Dr. Jaclyn Tolentino is a board-certified family medicine physician and Doctor of Osteopathy with over 15 years of experience, as well as a practitioner of integrative and functional medicine. She specializes in hormone optimization, metabolic health, immune resilience, and whole-person care, with a clinical approach grounded in both advanced medical science and the belief that the body has an incredible capacity to heal when given the right conditions to thrive.
As the founding Lead Physician at Love.Life, she joined the team before the doors even opened, drawn to the opportunity to help shape a new kind of healthcare experience. One that values collaboration, time, and true partnership between patient and provider. At Love.Life, she leads a multidisciplinary team of experts, including a registered dietitian, health coach, and acupuncturists who work together to craft personalized care plans that treat the whole person—not just symptoms. Dr. Tolentino is helping to shape a new standard for healthcare—one rooted in prevention, collaboration, and personalized healing.
A graduate of the Edward Via College of Osteopathic Medicine, Dr. Tolentino completed her residency at Westchester General Hospital and pursued additional training through the Institute for Functional Medicine. She has certifications in Advanced Bio-identical Hormone Replacement Therapy and Applied Quantum Biology. She previously served as a Senior Physician at Parsley Health. Her expertise has been featured in outlets like Vogue, The Wall Street Journal, Well+Good, MindBodyGreen, Women’s Health, and The Good Trade.
Dr. Tolentino’s story is one of bold pivots, personal healing, and a deep desire to redefine what health can look and feel like. She’s a young breast cancer survivor, an over 40 fertility success story, and a mom to her son Koa and their chihuahua, Cali. Outside of the clinic, she loves California sunsets, coffee walks with her husband Charles, and reading up on metabolic health just for fun. (Yes, she’s a nerd—always learning, always integrating the latest in wellness to better support her patients.)
At the heart of it all is one belief: when people feel seen, supported, and informed, they can reclaim their health—and their lives.
