Mobile Application Privacy Policy and Terms of Use
Effective Date: May 31, 2024
Introduction.
Welcome to the Love.Life mobile application (“mobile app”). Our mobile app is for consumers who are seeking medical services from Love.Life Medical and/or wellness services from Love.Life. and other users who input wellness information (collectively, “Users”). Our mobile app is available for download on IOS and Android mobile devices. Our mobile app connects to servers and systems owned, operated, and maintained in the United States by our business partners to provide Users with secure, mobile access to health, medical, and wellness information. We are committed to protecting your personal information and your right to privacy. This policy describes how we collect and use your information when you use our mobile app, and more generally, use any of our services (“Services”). The mobile app and Services will collectively be referred to as our “Platform.” We may update this policy at any time, and future updates are effective as soon as they are published.
The mobile app can consolidate data from your devices (such as Oura Ring, or Apple watch) and other applicable health records and apps you use, so you can have a more comprehensive view of your health information in one convenient place. You are in control over which features of the mobile app that you use, although some features of our mobile app may be enabled by default.
Who We Are.
The Love.Life community is owned by Healthy America, LLC (also referred to in this policy as “Company” “we,” “us,” and “our”) which includes Love.Life Medical PC, Healthy America El Segundo CA LLC, Love.Life Management LLC, and any other affiliated entities and subsidiaries, all of which are holders of your personal information.
Your Personal Information.
We do not sell your personal information. The ways in which we interact with your information in connection with our mobile app includes, without limitation:
- When you choose to input text, including adding a profile photo to our mobile app, we store a copy of your chosen photo and data.
- When you choose to view healthcare documents (such as letters or images) using our mobile app, to make the files viewable for you we temporarily store copies on your device in app-private storage. The temporary copies are deleted when you close your session on our mobile app.
- If you engage in a telehealth visit using our mobile app, when you join a visit with your provider, we will ask for permission to access your device’s video and audio functionality to make the telehealth visit possible. We do not record or store video or audio data from these visits.
- If you notify front desk staff electronically when you arrive for an appointment, you may choose to allow our mobile app to interact with your Bluetooth data for this purpose. We do not store your Bluetooth data.
- While you use our mobile app, if you choose to call a phone number displayed within the mobile app, we will ask for permission to access your device’s phone to place a call to the phone number. We do not store your call history or data about the call.
- While you use our mobile app, we collect identifying information so we can provide customer service to you.
- You may contact us at privacy@love.life. If you contact us, we may keep a record of the communication. You can decide how much information you want to share with us in those cases.
What Personal Information Do We Collect?
When you download our mobile app and create an account (“Account”), we may collect and store certain information about you, such as:
- Account Information. When you register with us using the mobile app to create an account and become a registered user, you will need to provide us with certain personally identifiable information to complete the registration, including information that can be used to contact or identify you and payment or other billing information in some cases.
- Device Information. We may automatically collect certain information about the device you use to access the Services. For example, we may collect and analyze information such as (a) IP addresses, geolocation information (as described in the next section below), unique device identifiers and other information about your mobile phone or other mobile device(s), browser types, browser language, operating system, the state or country from which you accessed the Services; and (b) information related to the ways in which you interact with the Services, such as: referring and exit pages and URLs, platform type, the number of clicks, domain names, landing pages, pages and content viewed and the order of those pages, the amount of time spent on particular pages, the date and time you used the Services, the frequency of your use of the Services, error logs, and other similar information. As described further below, we may use third-party analytics providers or service providers and technologies, including cookies and similar tools, to assist in collecting this information.
- Location Information. We may collect different types of information about your location, including general information (e.g., IP address, zip code) and more specific information (e.g., GPS-based functionality on mobile devices used to access the Services), and may use that information to customize the Services with location-based information, advertising, and features. In order to do this, your location information may be shared with our agents, third party vendors or third party advertisers. If you access the Services through a mobile device and you do not want your device to provide us with location-tracking information, you can disable the GPS or other location-tracking functions on your device, provided your device allows you to do this. See your device manufacturer’s instructions for further details.
- Cookies and Other Electronic Technologies. We may use the tools outlined below in order to better understand users. As we adopt additional technologies, we may also gather additional information through other methods.
- Cookies: “Cookies” are small computer files transferred to your computing device that contain information such as user ID, user preferences, lists of pages visited and activities conducted while using the Services. We use Cookies to help us improve or tailor the Services by tracking your navigation habits, storing your authentication status so you do not have to re-enter your credentials each time you use the Services, customizing your experience with the Services, and for analytics and fraud prevention.
- We may use a type of advertising commonly known as interest-based or online behavioral advertising, discussed in more detail below in the Section titled “Online Behavioral Advertising.” This means that some of our third-party business partners use Cookies to display Company ads on other websites and services based on information about your use of the Services and on your interests (as inferred from your online activity). Other Cookies used by our business partners may collect information when you use the Services, such as the IP address, mobile device ID, operating system, browser, web page interactions, the geographic location of your internet service provider, and demographic information such as sex and age range. These Cookies help Company learn more about our users’ demographics and internet behaviors.
- Platform Analytics: We may use third-party analytics services in connection with the Platform, including, for example, to register scrolling activity and text that you type into the Platform. These analytics services generally do not collect personal information unless you voluntarily provide it and generally do not track your browsing habits across sites which do not use their services. We use the information collected from these services to help make the Platform easier to use.
- Mobile Device Identifiers: Mobile device identifiers are data stored on your mobile device that may track mobile device and data and activities occurring on and through it, as well as the applications installed on it. Mobile device identifiers enable collection of personal information (such as media access control, address and location) and traffic data. Mobile device identifiers help Company learn more about our Users’ demographics and internet behaviors.
- Information from Third Parties. We may obtain additional information about you from third parties such as marketers, partners, researchers, and others. We may combine information that we collect from you with information about you that we obtain from such third parties and information derived from any other subscription, product, or service we provide.
- Aggregate or De-identified Data. We may aggregate and/or de-identify information collected by the Services or via other means so that the information is not intended to identify you. Our use and disclosure of aggregated and/or de-identified information is not subject to any restrictions under this Privacy Policy, and we may disclose it to others without limitation for any purpose, in accordance with applicable laws and regulations.
- Information collected from Apple watch and similar platforms. We may collect limited data from various applications such as, Oura Ring and Apple watch, among other platforms. In order to enhance our ability to provide better health care services, we access and utilize this data in a responsible and secure manner. Once the fitness activity data is retrieved from the platform, we securely store it in our own database to display trends over a period of time. Please note that health data obtained from Users’ wearable devices is for informational purposes only. We are not responsible for using this data for diagnostic purposes or for monitoring Users’ health. This data will not be monitored by your medical team at Love.Life. Users should not rely solely on data generated from wearables for medical decisions and should consult with their healthcare provider for any health concerns or conditions. Company disclaims any liability for the accuracy, completeness or reliability of data uploaded from wearable devices.
The following table lists the categories of Personal Information which Company collects or may collect. Company may collect this information from you or our service providers. Company will retain the Personal Information for so long as you are a User of the Service and in accordance with applicable laws before permanently deleting it.

An individual’s physiological, biological, or behavioral characteristics, including an individual’s deoxyribonucleic acid (DNA), that can be used, singly or in combination with each other or with other identifying data, to establish individual identity. Biometric information includes, but is not limited to, imagery of the iris, retina, fingerprint, face, hand, palm, vein patterns, and voice recordings, from which an identifier template, such as a faceprint, a minutiae template, or a voiceprint, can be extracted, and keystroke patterns or rhythms, gait patterns or rhythms, and sleep, health, or exercise data that contain identifying information.
Online Activity
Internet and other electronic network activity information, including, but not limited to, browsing history, search history, and information regarding your interaction with websites, applications, or advertisements
Geolocation Data
Physical location or movements.
Sensory Information
Audio, electronic, visual, thermal, olfactory or similar information.
Employment Information
Professional or employment-related information.
Inferences
Inferences drawn from any of the information identified above to create a profile about you reflecting your preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.
Sensitive Personal Information (as defined in Cal. Civ. Code § 1798.140)
Personal Information that reveals:
- A consumer’s social security, driver’s license, state identification card, or passport number.
- A consumer’s account log-in, financial account, debit card, or credit card number in combination with any required security or access code, password, or credentials allowing access to an account.
- A consumer’s precise geolocation.
- A consumer’s racial or ethnic origin, religious or philosophical beliefs, or union membership.
- The contents of a consumer’s mail, email, and text messages unless the business is the intended recipient of the communication.
- A consumer’s genetic data;
or
- The processing of biometric information for the purpose of uniquely identifying a consumer;
- Personal Information collected and analyzed concerning a consumer’s health; or
- Personal Information collected and analyzed concerning a consumer’s sex life or sexual orientation.
How Do We Use Collected Information?
We may use your Account information and other personal information for the following purposes:
- Fulfillment: We collect and use your personal information to operate mobile App and offer and provide our Services to you, including for the purposes for which you provided the information; to process your payments and other related customer service.
- Communications: We use your information to communicate with you, such as responding to your inquiries, providing you information, and for other customer service purposes. We will also send you mobile notifications unless you affirmatively opt-out of this service.
- Advertising: We also use your information to present advertising online or through other communication channels, including through partnerships with social media platforms and internet search engines.
- Mobile App Experience: We may use your information for the operation of the mobile App, to maintain the quality of our Platform, and to provide general statistics regarding use.
- Personalization: We may use your information to deliver customized content, personalized help and instructions, informing you of products, programs, events, services and promotions of Company, our affiliates and partners and other information and services that we believe may be of interest to you.
- Quality Assurance: For internal administrative and auditing purposes.
- Research: We may use your information to conduct research and measurement activities for purposes of product and service research and development, advertising claim substantiation, market research, and other activities related to Company, the mobile App and Services.
- Security: We may use your information to investigate fraud, security threats, and other harmful activity and protect our legal rights and to enforce our Terms and Conditions.
- Surveys: We may use your information to administer surveys and questionnaires.
- Aggregated or De-identified Data: We may aggregate and/or de-identify information collected by the mobile app and Services or via other means so that the information is not intended to identify you. Our use and disclosure of aggregated and/or de-identified information is not subject to any restrictions under this Privacy Policy, and we may disclose it to others without limitation for any purpose, in accordance with applicable laws and regulations.
- Disputes: We may use your information to resolve disputes, troubleshoot problems, to protect our rights or property, and to enforce our Terms and Conditions.
- Legal Obligations: We will use your information to respond to law enforcement requests, comply with legal and regulatory requirements and in responding to requests from courts or other government bodies, as required by applicable law.
Do We Keep Information Safe?
- We use technical controls and safeguards to protect the privacy, security, integrity, and availability of your personal information.
- We use https for secure communication between servers.
- When we store data on your mobile device, we store it in app-private storage that cannot be accessed by other apps.
- Before data is shared from our mobile app, we provide an in-app notification so you can choose if you want to share the data.
- We maintain internal policies and processes that limit access to your information to our staff who need to know the information to perform their jobs.
- We maintain internal data retention and deletion policies to help us ensure we only store information about your use of our mobile app as we describe in this policy.
However, despite our safeguards and efforts to secure your information, no electronic transmission over the Internet or information storage technology can be guaranteed to be 100% secure, so we cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security, and improperly collect, access, steal, or modify your information. Although we will do our best to protect your personal information, transmission of personal information to and from our Services is at your own risk. You should only access the Services within a secure environment.
You can take other steps to protect your information:
- Do not share the username and password that you use with third parties.
- Change your password immediately if you believe any unauthorized access has occurred.
- Use the security tools on your device that you use with our mobile app.
Confidentiality of Health Information
Health information that Company receives and/or creates about you, personally, relating to your past, present, or future health, treatment, or payment for healthcare services, may be “protected health information” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the Health Information Technology for Economic and Clinical Health Act (“HITECH Act”). Your health information may also be protected by state privacy laws and regulations.
Company understands that health information about you and your health is personal. We support your privacy and ensure that the transmittal and use of your information complies with all laws, except to the extent that you have authorized Company to transmit information to you by other means. In this regard, where applicable, we comply with HIPAA, HITECH, and other relevant state laws and regulations by entering into Business Associate Agreements with third party service providers to ensure that your protected health information is appropriately safeguarded. For additional information, review our Notice of Privacy Practices below.
Will Your Information Be Shared with Anyone?
We only share information with your consent, to comply with laws, to provide you with services, to protect your rights, or to fulfill business obligations. Our policy is not to disclose your protected health information or other personal information except in the limited circumstances described here:
· Service and Marketing Providers: We partner with certain trusted third parties to assist with the operation of our mobile app, including billing services, fulfilling requests, advertising, IT services, analyzing your interests and activity on our Platform, analytics and reporting, and other services on our behalf. These third parties may provide services to measure the effectiveness of our marketing campaigns and related to other purposes described above, and we may share your personal data (depending upon the nature of the service) with them, but only for the purposes of performing these functions and providing such services. We may also receive information collected by these third parties and combine it with the information we have collected. Your information may also be collected and processed by third parties, such as the payment providers you select, who will process your information independently in accordance with their own privacy notices.
· Healthcare Services: We may share your personal information with our affiliates and partners, including without limitation, our Business Associates pursuant to a written contract between us and every service provider. For instance, we have a business associate agreement with a provider who may fill supplement orders or other healthcare-related products orders for the purposes described in this Privacy Policy.
· Business Transfers: If (i) we or any of our affiliates undergoes a business transition or change of ownership, such as a merger or acquisition by another entity, or (ii) if any of our assets are or may be transferred to another entity, whether as part of a bankruptcy or insolvency proceeding or otherwise, we may be required to disclose or transfer the personal information we have collected from you. As part of the business transfer process, we may share certain of your personal information with lenders, auditors, and third-party advisors, including attorneys and consultants.
· Other Third Parties: We may disclose your personal information, without notice, if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on Company; (b) protect and defend the rights or property of Company; and, (c) act under exigent circumstances to protect the personal safety of users of Company or the public.
· With your consent.
We carry out due diligence on all service providers we engage to ensure they have adequate data protection and information security measures in place and only provide them with the personal data necessary for the service they are providing. In some cases, third parties may use the data they collect for their own purposes, for example, they may aggregate your data with other data they hold and use this to inform their services. Our service providers are subject to extensive obligations under contractual arrangements, including strict data retention limits.
How Long Do We Keep Your Information?
We keep your information for as long as necessary to fulfill the purposes outlined in this privacy notice unless a longer retention period is required or permitted by law (such as tax, accounting or other legal requirements). No purpose in this notice will require us keeping your personal information for longer than 10 years after you have ended your relationship with us. When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymize such information, or, if this is not possible (for example, because your personal information has been stored in backup archives), then we will securely store your personal information and isolate it from any further processing until deletion is possible.
Children’s Privacy
We do not knowingly solicit data from or market to children under 13 years of age. By using the Services, you represent that you are at least 13 or that you are the parent or guardian of such a minor and consent to such minor dependent’s use of the Services. If we learn that personal information from users less than 13 years of age has been collected, we will deactivate the account and take reasonable measures to promptly delete such data from our records. If you become aware of any data we may have collected from children under age 13, please contact us at privacy@love.life.
Controls for Do Not Track Signals
Most web browsers and some mobile operating systems and mobile applications include a Do-Not-Track (“DNT”) feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. At this stage, no uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online. If a standard for online tracking is adopted that we must follow in the future, we will inform you about that practice in a revised version of this privacy notice.
Your California Privacy Rights.
California’s “Shine the Light” law, Civil Code Section 1798.83, requires certain businesses to respond to requests from California customers asking about the business’s practices related to disclosing personal information to third parties for their direct marketing purposes. Users who are California residents may request and obtain from us, once a year and free of charge, information about categories of personal information (if any) we disclosed to third parties for direct marketing purposes and the names and addresses of all third parties with which we shared personal information in the immediately preceding calendar year. If you are a California resident and would like to make such a request, please submit your request in writing to us at privacy@love.life. If you are under 18 years of age, reside in California, and have a registered account with a Service, you have the right to request removal of unwanted data that you publicly post on the Services. To request removal of such data, please contact us using the contact information provided below, and include the email address associated with your account and a statement that you reside in California. We will make sure the data is not publicly displayed on the Services, but please be aware that the data may not be completely or comprehensively removed from all our systems (e.g. backups, etc.).
Your Privacy Choices
You can control the information we collect and use in the following ways:
- Location Information: You can disable location-based services on your mobile device or web browser by adjusting the settings on your device or browser. This will prevent our mobile app from accessing your location information. Note that some services may not be available if you disable location-based services.
- Emails: You can unsubscribe from marketing emails by clicking the unsubscribe link in the footer of such emails.
- Online Advertising: For information about opting out of third-party advertising, visit: Network Advertising Initiative’s (NAI) Consumer Opt-Out link, or the Digital Advertising Alliance’s (DAA) Consumer Opt-Out link (you will leave this Website for a separately managed online site where you can specify your preference under those programs). You can also click on the icon that may appear on some of our advertising served through these technologies. We may use more than one third-party company for placing this advertising, which would require you to opt out of each company. Please note that even after opting out of interest-based advertising, you may still see Company’s advertisements that are not interest-based (i.e., not targeted toward you). Also, opting out does not mean that Company is no longer using its tracking tools—Company still may collect information about your use of the Services even after you have opted out of interest-based advertisements and may still serve advertisements to you via the Services based on information it collects via those Services.
- Social Media: This privacy policy does not cover the privacy and security practices of social media platforms on which we have social media pages. Please review the terms, policies, and settings of those social media platforms if you have questions about how they collect and use your data.
- Third-Party Analytics. We use automated devices and applications to evaluate usage of our mobile app. We also may use other analytic means to evaluate our mobile app. We use these tools to help us improve our mobile app’s performance and user experiences.
Opt-In Policy
When you supply us with personally identifiable information in connection with your use of the Services, you may be asked to indicate whether you are interested in receiving information from us about our product and service offerings and if you would like us to share personally identifiable information about you with our affiliates, partners or other third parties for their marketing purposes. If you do choose to opt-in, you will receive such communications and/or we will share your information in accordance with your “opt-in” consent.
You may, of course, choose not to receive additional marketing information from us or choose not to allow our sharing of your personally identifiable information as follows: at any time, you can follow a link provided in our marketing-related email messages (but excluding e-commerce confirmations and other administrative emails) to opt out from receiving such communications; or at any time, you can contact us in accordance with the “Contact Us” section below to opt out from receiving such communications.
If you decide to contact us to change your contact preferences to opt out of receiving communications from us, please specify clearly which of the following choices you are opting out of: (a) receiving marketing communications from us; (b) allowing us to share personally identifiable information about you with our affiliates and partners for their marketing purposes; and/or (c) allowing us to share personally identifiable information about you with other third parties for their marketing purposes.
We will endeavor to implement your requested change as soon as reasonably practicable after receiving your request. Please be aware that your requested change will not be effective until we implement such change. Please note that if you choose not to allow our sharing of your personally identifiable information, we are not responsible for removing your personally identifiable information from the databases of third parties with which we have already shared your personally identifiable information as of the date that we implement your request. If you wish to cease receiving marketing-related e-mails from these third parties, please contact them directly or utilize any opt-out mechanisms in their privacy policies or marketing-related e-mails.
Please note that if you do opt-out of receiving marketing-related messages from us, we may still send you important administrative messages. You cannot opt-out from receiving these administrative messages. We reserve the right, from time to time, to contact former customers or users of the Services for administrative purposes or in order to comply with applicable laws, rules or regulations.

Our mobile app may enable you to navigate to third-party websites or other mobile apps through links on our mobile app. We do this for your convenience. We do not endorse, may not have any affiliation with, do not control, and are not responsible for those third-party websites and/or their web practices. We encourage you to review the privacy statements of websites or mobile apps you choose to link to from our mobile app so that you can understand how those websites collect, use, and share your information.
Notice of Privacy Practices.
Company is dedicated to maintaining the privacy of your protected health information (“PHI”). PHI is information about you that may be used to identify you (such as your name, social security number or address), and that relates to (a) your past, present or future physical or mental health or condition, (b) the provision of healthcare to you, or (c) your past, present, or future payment for the provision of healthcare. In conducting its business, Company may receive and create records containing your PHI. Company is required by law to maintain the privacy of your PHI and to provide you with notice of its legal duties and privacy practices with respect to your PHI.
Company must abide by the terms of this Notice while it is in effect. This Notice is in effect from the date noted above until Company replaces it. Company reserves the right to change the terms of this Notice at any time, as long as the changes are in compliance with applicable law. If Company changes the terms of this Notice, the new terms will apply to all PHI that it maintains, including PHI that was created or received before such changes were made. If Company changes this Notice, it will post the new Notice on its Platform and will make the new Notice available upon request.
Uses and Disclosures of PHI. Company may use and disclose your PHI in the following ways:
- Treatment, Payment and Healthcare Operations. Company is permitted to use and disclose your PHI for purposes of (a) treatment, (b) payment and (c) healthcare operations. For example:
- Treatment. Company may disclose your PHI to a physician in connection with the provision of treatment to you.
- Payment. Company may use and disclose your PHI to your health insurer or health plan in connection with the processing and payment of claims and other charges.
- Healthcare Operations. Company may use and disclose your PHI in connection with its healthcare operations, such as providing customer services and conducting quality review assessments. Company may engage third parties to provide various services for Company. If any such third party must have access to your PHI in order to perform its services, Company will require that third party to enter an agreement that binds the third party to the use and disclosure restrictions outlined in this Notice.
- Authorization. Company is permitted to use and disclose your PHI upon your written authorization, to the extent such use or disclosure is consistent with your authorization. You may revoke any such authorization at any time.
- As Required by Law. Company may use and disclose your PHI to the extent required by law.
- Special Circumstances. The following categories describe unique circumstances in which Company may use or disclose your PHI:
- Public Health Activities. Company may disclose your PHI to public health authorities or other governmental authorities for purposes including preventing and controlling disease, reporting child abuse or neglect, reporting domestic violence, and reporting to the Food and Drug Administration regarding the quality, safety and effectiveness of a regulated product or activity. Company may, in certain circumstances disclose PHI to persons who have been exposed to a communicable disease or may otherwise be at risk of contracting or spreading a disease or condition.
- Workers’ Compensation. Company may disclose your PHI as authorized by, and to the extent necessary to comply with, workers’ compensation programs and other similar programs relating to work-related illnesses or injuries.
- Health Oversight Activities. Company may disclose your PHI to a health oversight agency for authorized activities such as audits, investigations, inspections, licensing, and disciplinary actions relating to the healthcare system or government benefit programs.
- Judicial and Administrative Proceedings. Company may disclose your PHI, in certain circumstances, as permitted by applicable law, in response to an order from a court or administrative agency, or in response to a subpoena or discovery request.
- Law Enforcement. Company may, under certain circumstances, disclose your PHI to a law enforcement official, such as for purposes of identifying or locating a suspect, fugitive, material witness or missing person.
- Decedents. Company may, under certain circumstances, disclose PHI to coroners, medical examiners, and funeral directors for purposes such as identification, determining the cause of death and fulfilling duties relating to decedents.
- Organ Procurement. Company may, under certain circumstances, use or disclose PHI for the purposes of organ donation and transplantation.
- Research. Company may, under certain circumstances, use or disclose PHI that is necessary for research purposes.
- Threat to Health or Safety. Company may, under certain circumstances, use or disclose PHI if necessary to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
- Specialized Government Functions. Company, may in certain situations, use and disclose PHI of persons who are, or were, in the Armed Forces for purposes such as ensuring proper execution of a military mission or determining entitlement to benefits. Company may also disclose PHI to federal officials for intelligence and national security purposes.
Your Rights Regarding Your PHI. You have the following rights regarding the PHI maintained by Company :
1) Confidential Communication. You have the right to receive confidential communications of your PHI. You may request that Company communicate with you through alternate means or at an alternate location, and Company will accommodate your reasonable requests. You must submit your request in writing to Company.
2) Restrictions. You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment, or healthcare operations. You also have the right to request that Company limits its disclosures of PHI to only certain individuals involved in your care or the payment of your care. You must submit your request in writing to Company. Company is not required to comply with your request. However, if Company agrees to comply with your request, it will be bound by such agreement, except when otherwise required by law or in the event of an emergency.
3) Inspection and Copies. You have the right to inspect and copy your PHI. You must submit your request in writing to Company. Company may impose a fee for the costs of copying, mailing, labor, and supplies associated with your request. Company may deny your request to inspect and/or copy your PHI in certain limited circumstances. If that occurs, Company will inform you of the reason for the denial, and you may request a review of the denial.
4) Amendment. You have a right to request that Company amend your PHI if you believe it is incorrect or incomplete, and you may request an amendment for as long as the information is maintained by Company. You must submit your request in writing to Company and provide a reason to support the requested amendment. Company may, under certain circumstances, deny your request by sending you a written notice of denial. If Company denies your request, you will be permitted to submit a statement of disagreement for inclusion in your records.
5) Accounting of Disclosures. You have a right to receive an accounting of all disclosures Company has made of your PHI. However, that right does not include disclosures made for treatment, payment or healthcare operations, disclosures made to you about your treatment, disclosures made pursuant to an authorization, and certain other disclosures. You must submit your request in writing to Company and you must specify the time period involved (which must be for a period of time less than six years from the date of the disclosure). Your first accounting will be free of charge. However, Company may charge you for the costs involved in fulfilling any additional request made within a period of 12 months. Company will inform you of such costs in advance, so that you may withdraw or modify your request to save costs.
6) Breach Notification. You have the right to be notified in the event that Company (or a Company Business Associate) discovers a breach of unsecured PHI.
7) Paper Copy. You have the right to obtain a paper copy of this Notice from Company at any time upon request. To obtain a paper copy of this notice, please contact Company by calling (310) 424-7011.
8) Complaint. You may complain to Company and to the Secretary of the Department of Health and Human Services if you believe that your privacy rights have been violated. To file a complaint with Company, you must submit a statement in writing to Company at privacy@love.life . Company will not retaliate against you for filing a complaint.

To the extent you are required to send a written request to Company to exercise any right described in this Notice, you must submit your request to privacy@love.life.
