Step-by-step guide to scheduling a consultation outside the U.S.

Step 1: Request a portal account
Fill out the form on the Love.Life Telehealth home page and for your “State” select the last item, Outside the USA.

Step 2 – Activating your Patient Portal Account
Activating your Patient Portal account is simple. You will receive an email from Love.Life Telehealth <noreply@charmtracker.com> with the subject Patient Portal Registration. It contains the full instructions, including a help video, but all you really have to do is click the link in that email, and follow the on screen instructions to create a new password for your account.

If you already have a ChARM account, because another practice you visit uses the same system, you have to make sure to fully logout of ChARM first, and then go back to the invitation to click the link again to start the process of adding Love.Life Telehealth to your existing ChARM account. You will not need to create a second account, rather when you login you will choose which practice you want to login to.

Step 3 – Requesting an Appointment
There are two ways to request an appointment, and both are easy, so please choose whichever is more convenient for you.

Note: All doctors at Love.Life Telehealth are available for International Consultations, so feel free to choose whichever doctor you like.

When making your payment, if prompted to enter an address and US zip code, and if unable to enter your international address, you are welcome to enter our practice address to complete payment. Our address is 1716 Main Street, Suite 163, Longmont, CO 80501.

From the Meet The Doctors page on our web site:

  1. Visit the meet the doctors web page at https://love.life/telehealth/find-your-doctor/
  2. Click the doctors picture that you would like to make an appointment with to scroll to their bio.
  3. Click the “SCHEDULE APPOINTMENT” button
  4. Verify the doctor is licensed to treat you in your state
  5. Select an appointment type (new patient or follow-up, and 30 or 60 minutes)
  6. Use the calendar to select an open time that fits your schedule
  7. Complete the form
  8. Enter your payment information (or select card on file)
  9. Click CONFIRM

From the Patient Portal

  1. Login to the ChARM patient portal here
  2. Select REQUEST APPOINTMENT at the top center, in the blue bar
  3. Select the facility for your doctor (by their name), and then select the doctor (their name again)
  4. Select an appointment type (new patient or follow-up, and 30 or 60 minutes)
  5. Enter the reason for your appointment, and payment information (or select card on file)
  6. Click REQUEST, at the bottom right

That's it, your appointment will be scheduled and you will see it listed under the appointment option in your patient portal.

Step 4 – Complete your assigned questionnaires
Your doctor will assign questionnaires to you based on the type of visit (new patient or follow-up), and your specific needs (food diary, and others). Please complete these questionnaires 24 hours before your appointment, to give your doctor time to review them ahead of time. Doing so will enable you to have an efficient and productive appointment with your doctor.

To view and complete your assigned questionnaires:

  1. Login to the ChARM patient portal here
  2. Click the Questionnaire icon
  3. Click the questionnaire name (Informed Consent, New Patient Intake, 3-Day Food Diary, etc.)
  4. Complete the questionnaire
  5. Go on to the next one!

Step 5 – Checking in for your Appointment
Checking in for your appointment is simple. Just follow the steps below to login to your patient portal, list your upcoming appointment, and click the CHECK-IN link with the camera icon. This will open up Zoom, your doctor will join at your appointment time, and you will have an audio and video visit with your doctor.

  1. Login to the ChARM patient portal here
  2. Click the Appointments icon (looks like a calendar and watch)
  3. Click CHECK-IN, next to your scheduled appointment date and time

Step 6 – Uploading Lab Results and Documents
Uploading prior labs, logs, and other documents into your patient portal will help your doctor to have a more in-depth understanding of your current health condition. To upload these files, just follow these steps.

  1. Login to the ChARM patient portal here
  2. Click the Documents icon
  3. Click Upload, and browse to find your file, or multiple files
  4. Click Upload and Share
  5. Select the provider, or providers you would like to share the documents with
  6. Click Share

That's all there is to it. You will be able to the files you uploaded listed in your documents. On the left hand side you can click Shared By Me to see which of your documents have been shared, the date they were shared, and who has access to see them.

You can also hover your mouse over a document to share it after it has been uploaded, or to download it back to your own computer, or to delete it.
